The overall purpose of the Social Services Director position is to enable the facility to identify the medically related social and emotional needs of the residents: This position acts to meet those needs through individual and group social services programming. This position also promotes residents’ potential during their stay and collaborates with medical personnel and resident in preparation for discharge.
Regulatory Requirements:
- Bachelor degree in social work (BSW), (or secondary education in the social services and certification as a “social work designee” may be substituted as appropriate).
- Knowledgeable with Medicare, Medicaid, Insurance, PASRRs, MDS, and Care Plans
- Two (2) or more years of experience in all aspects of social services and discharge planning in a long term care setting
- Ability to integrate and coordinate social service/discharge planning activities are coordinated within the hospital settings
- Ability to perform a psycho social assessment and reassessment on all residents
- Coordinate and direct social service/discharge planning department to ensure resident needs are met
- Provide appropriate resources and support to social service/discharge planning staff/clinical staff
- Maintain regulatory requirements, including all federal state local regulations and accrediting organization standards
