The position of Healthcare Concierge will coordinate new admissions to the facility and function as the resident liaison by responding to customer service requests and resolving problems. This position will ultimately be responsible for personally enriching the lives of the people we serve and enhancing our overall customer service. The Healthcare Concierge helps provide a comfortable transition to the facility and smooth admissions process. The responsibilities of the position will range widely based on the needs of the client. This position requires you to be flexible and accommodating. Responsibilities will often include doing many daily tasks.
Requirements:
- Must be a CNA
- 2 years of professional experience, preferably in a long term care environment
- Knowledge preferred of Medicare and Medicaid requirements and procedures, as well as laws, regulations and guidelines that pertain to long term care
- Basic computer literacy and skills
- Exceptional communication, customer service and people skills
- Outgoing, friendly personality
- Good organizational skills, follow up and follow through
- Ability to handle multiple tasks at one time
- Self-motivated, can work independently
- High desire to work with the elderly
- Must be able to deal tactfully with team members, other staff members, residents, family members, visitors, government agencies and the general public

Responsibilities:
- Greeting new admissions at time of arrival to include hanging up clothing, ensuring all clothing is labeled, putting all other items away, offering a meal, removing other arm bands and assisting with clothing change if needed
- Provide all contact information for facility to include: facility phone number and address, leadership information, physician contact information etc.
- Complete admission paperwork within 48 hours of admission
- Daily rounds to check all empty rooms daily to ensure they are ready for new admissions
- Work with housekeeping, maintenance and central supply for new room readiness
- Placing, monitoring, and auditing door tags and arm bands (if applicable) for all residents
- Assist the clinical team in dressing/bathing as needed
- Assist with activities as needed
- Act as a liaison between clinical, social services, activities and admissions to meet the needs of the resident
- Assist with room changes in order to accommodate new admissions
- Follow up daily with all new admissions for three weeks
- Assist resident with needs to include packing at time of discharge
- Ability to understand, monitor, and perform all internal admission processes
- Work effectively and efficiently under pressure and within time constraints
- Participating in in-service educational activities and department meetings
- Performing all other duties as assigned
