Notice of Breach:
Due to the nature of the breach, we feel that there is minimal risk of future impact, as the information included in the breach DID NOT include Social Security Numbers or Dates of Birth.
What Happened: On August 14, 2020 it was discovered that protected health information was lost and unable to be accounted for.
What information was involved: Notes included resident names and room numbers
