New Dialysis Services offered through Concerto partnership!

St. Sophia Health & Rehabilitation Center and The Quarters at Des Peres is excited to announce that we have partnered with Concerto Renal Services to offer on-site dialysis services for our residents! These are life preserving services that will make a huge impact on the quality of life, comfort and health of our residents.

Offering dialysis services means our residents can receive more frequent treatments as ordered by their physicians. This lowers the risk of health complications, can lead to fewer food restrictions and medications, and just helps patients feel better! Providing this support means more control, freedom and flexibility over their care and over their schedules…and ultimately more time to do the activities they love. Learn More about Concerto!

THE INDUSTRY LEADER IN NURSING HOME DIALYSIS
Founded by seasoned nursing home experts, Concerto understands the needs of nursing home operators. This knowledge, combined with a clinical team of leading dialysis specialists, has positioned Concerto as the industry leader in Nursing Home Dialysis and your preferred partner in care.
Approximately 70,000 skilled nursing facility (SNF) residents in the United States are diagnosed with end-stage renal disease (ESRD). Providing effective dialysis care to nursing home residents presents unique challenges for operators. The SNF population is highly decentralized, presenting various logistical challenges and necessitating innovation relating to technological integration, coordination of care, and clinical oversight. With Concerto’s innovative model, these challenges are overcome, resulting in an improved process and healthier outcomes for SNF dialysis patients.

BENEFITS OF NURSING HOME DIALYSIS
Continuity of care: Doctors, nurses, physical and occupational therapists, renal dietitians, renal social workers and dialysis clinical staff members all work together in harmony to create a customized care plan to meet each patient’s needs.
Continuous, stable nursing care: Concerto creates a schedule allowing the same nurse to be in the dialysis unit for each treatment, promoting a level of familiarity and comfort for patients.
Convenience: Patients avoid hours of transportation back and forth to the hospital and exposure to inclement weather and other transportation risks.
Rehabilitation: With more efficient use of the patient’s time, patients focus on getting better.

CONCERTO LEADING THE WAY
Concerto’s clinical leadership team includes nephrology experts who have a deep understanding of the inner workings of SNFs. As a pioneer in nursing home dialysis, Concerto has quickly grown to become the largest and most qualified provider of nursing home dialysis in the United States. Our growth is attributable to our proprietary care model, designed specifically for geriatric ESRD patients residing in SNFs.
