<p>The Certified Nursing Assistant (CNA) position is to provide each of the assigned residents with routine daily nursing care and services in accordance with the resident’s care plan.</p>

<div>Provide evening care which includes hands/face washing as needed, oral hygiene, back rubs, peri-care, freshening linen, cleaning over-bed tables, straightening room and other general care as needed.</div>

<div>Report and record any changes observed in condition or behavior of residents and unusual incidents.</div>

<div>Advise supervising staff of resident’s status at beginning and end of shifts.</div>

<div>Assist RN and LPN with treatment procedures.</div>

<div>Prepare resident for meals; serve and remove food trays and assist with meals or feeding resident if necessary.</div>

<div>Assist Resident with the handling of personal property:  Dentures, glasses, contact lenses, hearing aids, prosthetic devices, etc.</div>

<div>Distribute drinking water and other nourishments to residents.</div>

<p><b>Director of Nursing &#8211; DON Responsibilities:</b></p>
<ul>
<li>Provide personal care to residents in a manner conductive to their safety and comfort consistent with Company Clinical Policies and Procedures as well as state/federal guidelines and regulations.</li>
<li>Initiates the development and implementation of policies and procedures that govern the nursing staff in collaboration with appropriate services.</li>
<li>Responsible for the provision of nursing services 24 hours a day, seven days a week.</li>
<li>Establish and maintain job specification and descriptions for nursing staff.</li>
<li>Address and support cultural practices as long as such practices do not harm others or interfere with the planned course of medical therapy.</li>
<li>Recommend modifications, additions, or deletions to nursing staffing plans to ensure reasonable hours and acceptable working conditions to provide optimal resident care coverage.</li>
<li>Assure that each resident’s accident or incident is fully documented and reported in accordance with Company Clinical Policies and Procedures and state/federal guidelines and regulations.</li>
<li>Represent nursing staff on various committees, Infection Prevention and Control, Pain Management, Performance Improvement, and Quality Assurance.</li>
<li>Make rounds to all nursing units daily.</li>
<li>Participate in performance improvement functions by collecting and verifying data, as needed.</li>
<li>Consult other departments, as appropriate, to provide for an interdisciplinary approach to the resident’s needs.</li>
<li>Ensure licensed nurses are able to perform head-to-toe assessments and reassessments on all residents.</li>
<li>Demonstrate knowledge of the geriatric residents and evaluate licensed nurse’s abilities to asses and reassess residents.</li>
<li>Ability to interpret results of test and procedures; take appropriate action.</li>
<li>Assess residents on admission and at least __________, as well as when conditions change and document appropriately.</li>
<li>Ability to adequately assess and reassess residents pain, utilize appropriate pain management techniques, and education resident and family on pain management.</li>
<li>Interpret test results and take appropriate action on results.</li>
<li>Promote and support the greatest possible degree of independence for residents.</li>
<li>Contribute knowledge of residents’ conditions and document observations as required.</li>
<li>Promote a safe and clean hazard-free environment for residents, family members, and staff members.</li>
<li>Assure equipment is clean and functional. Report any malfunctions to supervisor.</li>
<li>Ability to function as a team leader and role model.</li>
<li>Abide by the Company Clinical policies and procedures for Abuse Prevention.</li>
<li>Notify physician of changes in resident’s condition and follow through until appropriate action is taken.</li>
<li>Follow through, as needed, on information given by shift report, resident or family regarding resident concerns.</li>
<li>Implement plan of care for the resident based on assessments and goals as established by the interdisciplinary care team.</li>
<li>Attend all required meetings.</li>
<li>Plan and coordinate with Administrator, utilizing the respective service managers for planning the budgeting requirements for staff, performance of work, supplies, and equipment.</li>
<li>Responsible for cost controls to insure maximum effectiveness of funds expended from approved department budgets.</li>
<li>Follows infection prevention and control procedures and supervises supportive staff in infection prevention and control procedures.</li>
<li>Ability to react decisively and quickly in emergency situations.</li>
<li>Perform other task and duties as assigned.</li>

<p>We are looking for a Medication Tech to work a 12-hour Shift.</p>
<p><strong>Certified Medication Tech (CMT) Summary:</strong></p>
<ul>
<li>The overall purpose of the Certified Medication Tech (CMT) position is to administer oral and topical medications to residents.</li>
<li>This individual must perform these tasks as ordered by a physician, under the supervision of a licensed nurse, and in accordance with state and federal rules and regulations.</li>
<li>Must perform routine daily tasks required in maintaining the health, comfort and well-being of the residents.</li>
</ul>
<p><strong>Certified Medication Tech (CMT) Requirements:</strong></p>
<ul>
<li>CMT Certification</li>
<li>Able to communicate effectively in English, both verbally and in writing.</li>
<li>Additional languages preferred.</li>
<li>Basic computer knowledge.</li>
</ul>
<p><strong>Certified Medication Tech (CMT) Duties and Responsibilities:</strong></p>
<ul>
<li>Performs Nightly Audits, filing Medical records.</li>
<li>Charge CMT will manage CNA assignments.</li>
<li>Performs resident care responsibilities in a manner conducive to their safety and comfort consistent with MGM Healthcare Clinical Policies and Procedures as well as state/federal guidelines and regulations.</li>
<li>Administers medications as ordered by a physician under the supervision of a licensed nurse, in accordance with any state and federal regulations and consistent with facility policy.</li>
<li>Must demonstrate knowledge of “five rights” of medication administration. Administers and records all medication given to residents&#8217; based upon their specific chart.</li>
<li>Notifies appropriate licensed staff when a resident complains of pain.</li>
<li>May take and record vital signs of residents as assigned or as may be required by certain medications.</li>
<li>Records the administration of medications and/or treatments appropriately, utilizing the computer system to enter and reference resident information as needed.</li>
<li>Answers resident call lights, anticipates resident&#8217;s needs and makes rounds to assigned resident under the supervision of a licensed nurse or charge nurse on the shift.</li>
<li>Observes for and reports any reactions and/or side effects of medications to a licensed nurse on the appropriate shift.</li>
<li>Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.</li>
<li>Support and maintains a culture of safety and quality.</li>
<li>Administers PRN medications in accordance with state and federal regulations and consistent with facility policy.</li>
<li>Reports and when appropriate, records any changes observed in condition or behavior of residents and unusual incidents.</li>
<li>Performs other duties as assigned.</li>
</ul>

<p><strong>Responsibilities:</strong></p>
<ul>
<li>Reports to the professional nurse when leaving and returning to unit</li>
<li> Monitor stock of Central supplies and restock clean Utility rooms as needed</li>
<li> Assist patients in maintaining optimum nutritional status by hydration pass</li>
<li> Assist residents to and from dining room.</li>
<li> Assist staff in dining room by passing trays and preparing (cutting) food.</li>
<li> Work with dining room personnel to ensure that all residents get the correct meals and that all residents are able to eat</li>
<li>Makes observational rounds as directed by the charge nurse.</li>
<li>Engage in housekeeping tasks such as replacing linens on beds, cleaning patient rooms and removing and replacing trash bags</li>
<li>Contact Residents family members as requested</li>
<li> Wheelchair cleaning</li>
<li> Accompany residents as needed to various activities and functions.</li>
<li>Complete required documentation accurately and in a timely manner.</li>
<li> Demonstrate understanding of and utilize proper infection control policies/practices.</li>
<li>Maintain a clean, safe and secure environment for patients.</li>
<li> Act in compliance with all corporate, state, federal and other regulatory standards.</li>
<li> Attend meetings and in-service sessions as directed.</li>
<li>Notifies professional nurse as soon as possible of situations or circumstances occurring which may interfere with the completion of assigned duties within the designated shift.</li>
<li> Participate in the quality improvement process by responding appropriately to results of medical records audits, patient satisfaction requirements.</li>
<li>Will participate in the facility safety program and assist in promoting the awareness of safety issues.</li>
<li>Perform other duties as assigned by Nurse Managers to meet the needs of the residents.</li>
