<p>The overall purpose of the Certified Medication Tech (CMT) position is to administer oral and topical medications to residents.<br />
This individual must perform these tasks as ordered by a physician, under the supervision of a licensed nurse, and in accordance with state and federal rules and regulations.<br />
Must perform routine daily tasks required in maintaining the health, comfort and well-being of the residents.</p>
<p><strong>Certified Medication Tech (CMT) Requirements</strong>:</p>

<p><strong>Certified Medication Tech (CMT) Duties and Responsibilities:</strong></p>
<ul>
<li>Performs resident care responsibilities in a manner conducive to their safety and comfort consistent with MGM Healthcare Clinical Policies and Procedures as well as state/federal guidelines and regulations.</li>
<li>Administers medications as ordered by a physician under the supervision of a licensed nurse, in accordance with any state and federal regulations and consistent with facility policy.</li>
<li>Must demonstrate knowledge of “five rights” of medication administration. Administers and records all medication given to residents’ based upon their specific chart.</li>
<li>Notifies appropriate licensed staff when a resident complains of pain.</li>
<li>May take and record vital signs of residents as assigned or as may be required by certain medications.</li>
<li>Records the administration of medications and/or treatments appropriately, utilizing the computer system to enter and reference resident information as needed.</li>
<li>Answers resident call lights, anticipates resident’s needs and makes rounds to assigned resident under the supervision of a licensed nurse or charge nurse on the shift.</li>
<li>Observes for and reports any reactions and/or side effects of medications to a licensed nurse on the appropriate shift.</li>
<li>Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.</li>
<li>Support and maintains a culture of safety and quality.</li>
<li>Administers PRN medications in accordance with state and federal regulations and consistent with facility policy.</li>
<li>Reports and when appropriate, records any changes observed in condition or behavior of residents and unusual incidents.</li>

<li>Must have extensive clinical experience, prior administrative experience, and training in both nursing and management.</li>
<li>Must have strong communication skills, diplomacy and the ability to take leadership of any situation.</li>
</ul>
<p><strong>Unit Manager Supervisory Duties:</strong></p>
<ul>
<li>Nurse Unit Managers handle all supervisory duties for the unit, overseeing registered and licensed practical nurses, nursing aides, medical clerks and support staff.</li>
<li>They set work schedules, delegate assignments, assign tasks and evaluate employee job performance, as well as disciplining employees who don’t fulfill their job requirements or provide inadequate patient care.</li>
<li>They also establish employee policies and procedures.</li>
<li>Nurse Unit Managers set goals and standards for the unit.</li>

<p><b>Job Description</b></p>
<p>The Licensed Practical Nurse (LPN) position is to work under immediate supervisor and support providers in provision of patient care, patient procedures, and patient documentation.</p>
<p><b>Essential Functions</b></p>
<ul>

<li>Initiate and lead individualized care plans.</li>
<li>Promote and support the greatest possible degree of independence for residents.</li>
<li>Demonstrate the ability to perform a head-to-toe assessment on all residents. This includes pediatric, geriatric, and general patient population.</li>
<li>Demonstrate the ability to revise care plan as indicated by the residents response to treatment and evaluate overall plan daily for effectiveness.</li>
<li>Advise supervising staff of resident’s status at beginning and end of shifts.</li>
<li>Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills under the direction of an RN.</li>
<li>Contribute knowledge of residents’ conditions and document observations as required.</li>
<li>Promote a safe and clean hazard-free environment for residents, family members, and staff members.</li>
<li>Assure equipment is clean and functional. Report any malfunctions to supervisor.</li>
<li>Ability to function as a team leader and role model.</li>
<li>Abide by the Spring Valley Health and Rehabilitation Center Clinical policies and procedures for Abuse Prevention.</li>
<li>Accurately and promptly implement physician orders.</li>
<li>Order prescriptions and administer medications.</li>
<li>Demonstrate knowledge of medication and their correct administration based on age and clinical condition of resident.</li>
<li>Ability to react decisively and quickly in emergency situations.</li>
<li>Perform other task and duties as assigned.</li>
</ul>
