										<content:encoded><![CDATA[<p>The overall purpose of the Administrator position is to direct the overall operation of the facility in accordance with applicable local, state, and federal guidelines and regulations while ensuring quality care is delivered.</p>
<p><strong>As the Administrator, you will:</strong></p>
<ul>
<li>Be responsible for the operation of the facility</li>
<li>Make daily routine rounds throughout the facility to assure from a first – hand observation that an optimum level of resident care is provided, the environment is being maintained in a safe and sanitary fashion, and that employees are appropriately attired and receiving appropriate supervision</li>
<li>Implement all policies established by the MGM Healthcare; advises during the formation of such policies and reports on the implementation of such policies to MGM Leadership</li>
<li>Develop an annual budget to be prepared showing the expected revenue and expenditures as required by the MGM leadership</li>
<li>Develop, establish, and maintain resident census at optimal levels</li>
<li>Prepare an annual operating budget for approval and achieve at least budgeted earnings expectations</li>
<li>Review operating results of the organization, and compare them to established objectives and take steps to ensure that appropriate measures are taken to correct any unsatisfactory results</li>
<li>Select, employ, control, and discharge staff</li>
<li>Develop and maintain personnel policies and practices of the facility</li>
<li>Oversee recruitment, evaluations, selection, and training of competent department supervisors and other ancillary staff</li>
<li>Supervise the business affairs of the facility to ensure that funds are collected and expended to the best possible advantage</li>
<li>Ensure all contractual agreements are approved by the Corporate Office</li>
<li>Present MGM Leadership with reports that reflect the services and financial activities of the facility and other required special reports</li>
<li>Ensure that the facility maintains accreditation, licensing and quality resident care through the establishment of quality / performance improvement monitoring programs and standards</li>
</ul>
<p><strong>The Administrator will demonstrate competency in the following areas:</strong></p>
<ul>
<li>Knowledge of resident’s rights in long term care facilities and able to impart this knowledge to the staff</li>
<li>Able to maintain a culture of safety and quality</li>
<li>Able to maintain timely records and documentation appropriate to Medicaid/Title XIX/Medicare/other reimbursement programs</li>
</ul>
<p><strong>Requirements:</strong></p>
<ul>
<li>Bachelor’s Degree in Health Care Administration or other related field; Master’s preferred</li>
<li>Current and valid state Nursing Home Administrator license. Will consider a candidate that is soon to be licensed.</li>
<li>Able to communicate effectively in English, both verbally and in writing</li>
<li>Basic computer knowledge</li>
</ul>

<p><b>Director of Nursing &#8211; DON Responsibilities:</b></p>
<ul>
<li>Provide personal care to residents in a manner conductive to their safety and comfort consistent with Company Clinical Policies and Procedures as well as state/federal guidelines and regulations.</li>
<li>Initiates the development and implementation of policies and procedures that govern the nursing staff in collaboration with appropriate services.</li>
<li>Responsible for the provision of nursing services 24 hours a day, seven days a week.</li>
<li>Establish and maintain job specification and descriptions for nursing staff.</li>
<li>Address and support cultural practices as long as such practices do not harm others or interfere with the planned course of medical therapy.</li>
<li>Recommend modifications, additions, or deletions to nursing staffing plans to ensure reasonable hours and acceptable working conditions to provide optimal resident care coverage.</li>
<li>Assure that each resident’s accident or incident is fully documented and reported in accordance with Company Clinical Policies and Procedures and state/federal guidelines and regulations.</li>
<li>Represent nursing staff on various committees, Infection Prevention and Control, Pain Management, Performance Improvement, and Quality Assurance.</li>
<li>Make rounds to all nursing units daily.</li>
<li>Participate in performance improvement functions by collecting and verifying data, as needed.</li>
<li>Consult other departments, as appropriate, to provide for an interdisciplinary approach to the resident’s needs.</li>
<li>Ensure licensed nurses are able to perform head-to-toe assessments and reassessments on all residents.</li>
<li>Demonstrate knowledge of the geriatric residents and evaluate licensed nurse’s abilities to asses and reassess residents.</li>
<li>Ability to interpret results of test and procedures; take appropriate action.</li>
<li>Assess residents on admission and when conditions change and document appropriately.</li>
<li>Ability to adequately assess and reassess residents pain, utilize appropriate pain management techniques, and education resident and family on pain management.</li>
<li>Interpret test results and take appropriate action on results.</li>
<li>Promote and support the greatest possible degree of independence for residents.</li>
<li>Contribute knowledge of residents’ conditions and document observations as required.</li>
<li>Promote a safe and clean hazard-free environment for residents, family members, and staff members.</li>
<li>Assure equipment is clean and functional. Report any malfunctions to supervisor.</li>
<li>Ability to function as a team leader and role model.</li>
<li>Abide by the Company Clinical policies and procedures for Abuse Prevention.</li>
<li>Notify physician of changes in resident’s condition and follow through until appropriate action is taken.</li>
<li>Follow through, as needed, on information given by shift report, resident or family regarding resident concerns.</li>
<li>Implement plan of care for the resident based on assessments and goals as established by the interdisciplinary care team.</li>
<li>Attend all required meetings.</li>
<li>Plan and coordinate with Administrator, utilizing the respective service managers for planning the budgeting requirements for staff, performance of work, supplies, and equipment.</li>
<li>Responsible for cost controls to ensure maximum effectiveness of funds expended from approved department budgets.</li>
<li>Follows infection prevention and control procedures and supervises supportive staff in infection prevention and control procedures.</li>
<li>Ability to react decisively and quickly in emergency situations.</li>
<li>Perform other task and duties as assigned.</li>

<p>Attention to detail and the ability to follow instructions for reading recipes, dietary needs and meal orders.You must be physically able to prepare food, serve meals, as well as be able to operate kitchen equipment and clean up after meals</p>
<h5><strong>Cook Responsibilities:</strong></h5>
<ul>
<li>Prepare the food for the residents</li>
<li>Assist in clean up, kitchen maintenance and setting up and taking down the dining areas</li>
<li>Other job duties include delivering food to residents&#8217; rooms, preparing and serving snacks and taking inventory of food and kitchen supplies.</li>
<li>Cook must learn the protocols and procedures for their facilities, as well as the rules and regulations for the states where they work.</li>
</ul>
<h5><strong>Cook Qualifications:</strong></h5>
<ul>
<li>High school diploma or GED</li>
<li>Previous experience as a Dietary Cook preferred</li>
<li>Along with physical skills, a cook must be compassionate and patient when working with elderly residents, as well as the residents&#8217; family members.</li>

<p>The overall purpose of the Certified Medication Aide (CMA) position is to administer oral and topical medications to residents. This individual must perform these tasks as ordered by a physician, under the supervision of a licensed nurse, and in accordance with state and federal rules and regulations. Also, must perform routine daily tasks required in maintaining the health, comfort and well-being of the residents.</p>
<p><strong>Certified Medication Aide (CMA) Requirements:</strong></p>
<ul>
<li>CMT Certification</li>
<li>Able to communicate effectively in English, both verbally and in writing.</li>
<li>Additional languages preferred.</li>
<li>Basic computer knowledge.</li>
</ul>
<p><strong>Certified Medication Aide Duties and Responsibilities:</strong></p>
<ul>
<li>Performs resident care responsibilities in a manner conducive to their safety and comfort consistent with MGM Healthcare Clinical Policies and Procedures as well as state/federal guidelines and regulations.</li>
<li>Administers medications as ordered by a physician under the supervision of a licensed nurse, in accordance with any state and federal regulations and consistent with facility policy.</li>
<li>Must demonstrate knowledge of “five rights” of medication administration. Administers and records all medication given to residents&#8217; based upon their specific chart.</li>
<li>Notifies appropriate licensed staff when a resident complains of pain.</li>
<li>May take and record vital signs of residents as assigned or as may be required by certain medications.</li>
<li>Records the administration of medications and/or treatments appropriately, utilizing the computer system to enter and reference resident information as needed.</li>
<li>Answers resident call lights, anticipates resident&#8217;s needs and makes rounds to assigned resident under the supervision of a licensed nurse or charge nurse on the shift.</li>
<li>Observes for and reports any reactions and/or side effects of medications to a licensed nurse on the appropriate shift.</li>
<li>Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.</li>
<li>Support and maintains a culture of safety and quality.</li>
<li>Administers PRN medications in accordance with state and federal regulations and consistent with facility policy.</li>
<li>Reports and when appropriate, records any changes observed in condition or behavior of residents and unusual incidents.</li>
<li>Performs other duties as assigned.</li>

<p><strong>Certified Nursing Assistant (CNA) Requirements:</strong></p>
<ul>
<li>CNA Education: High School diploma or equivalent preferred.</li>
<li>Must pass a state-issued exam</li>
<li>CNA Licenses: Required Valid certification as a CNA in the state of Iowa.</li>
<li>Certified Nursing Assistant Experience: Six months experience in a long term care environment preferred.</li>
</ul>
<p><strong>Certified Nursing Assistant (CNA) Essential Functions:</strong></p>
<ul>
<li>Answers signal lights and bells to determine resident needs</li>
<li>Assists residents with grooming, oral hygiene, bathing, hair and incontinence care</li>
<li>Obtains food trays and assists residents with feeding</li>
<li>Assists resident with range of motion exercises</li>
<li>Provides for personal care and comfort</li>
<li>Movement to wheelchair or activity areas</li>
<li>Assists residents with ambulation for short distance in facility</li>
<li>Takes and records temperature, blood pressure, pulse, and respiration rates, and food and fluid intake and output, as directed</li>
<li>Provides direct and indirect resident care services</li>
<li>Meets the communication needs of resident and family</li>
<li>Communicates to nursing team changes in residents clinical condition</li>
<li>Performs other tasks as assigned</li>
